About the RoleÂ
The Care Coordinator is the bridge between clients, families, and care staff, ensuring smooth intake, scheduling, and service delivery.Â

Conduct intake consultations and assessmentsÂ

Develop and update care plansÂ

Coordinate staff scheduling and coverageÂ

Communicate with families and referral partnersÂ

Maintain accurate documentation and recordsÂ
Key ResponsibilitiesÂ

Background in health administration, social work, or care coordinationÂ

Strong organizational and communication skillsÂ

Experience in home care or community services (asset)Â Â

Comfort with scheduling software and documentationÂ
