About the Role
The Care Coordinator is the bridge between clients, families, and care staff, ensuring smooth intake, scheduling, and service delivery.

Conduct intake consultations and assessments

Develop and update care plans

Coordinate staff scheduling and coverage

Communicate with families and referral partners

Maintain accurate documentation and records
Key Responsibilities

Background in health administration, social work, or care coordination

Strong organizational and communication skills

Experience in home care or community services (asset)

Comfort with scheduling software and documentation
