Care Coordinator / Intake Specialist

Location: Remote / Hybrid (Nova Scotia-based preferred)
Employment Type: Part-time | Casual 

About the Role 

The Care Coordinator is the bridge between clients, families, and care staff, ensuring smooth intake, scheduling, and service delivery. 

  • Conduct intake consultations and assessments 

  • Develop and update care plans 

  • Coordinate staff scheduling and coverage 

  • Communicate with families and referral partners 

  • Maintain accurate documentation and records 

Key Responsibilities 

  • Background in health administration, social work, or care coordination 

  • Strong organizational and communication skills 

  • Experience in home care or community services (asset)  

  • Comfort with scheduling software and documentation
