About the Role
This role builds awareness and partnerships with hospitals, community organizations, and referral sources.

Develop relationships with hospitals and community partners

Support referral processes

Represent Grace Harbour Home Health at community events

Coordinate outreach materials and presentations
Key Responsibilities

Experience in community engagement or partnerships

Strong communication and relationship-building skills

Knowledge of Nova Scotia’s health and community landscape (asset)
